1.1 Implantatmaterialien und ihre Bindung an das Implantatlager Die Entwicklung der orthopadischen Chirurgie ist in den vergangenen 2030 J ahren durch die standig zunehmende Implantation von Fremdmaterialien gekennzeichnet. Zum einen werden diese in Form von Nageln, Schrauben und Platten temporar implantiert, zum anderen wird jedoch auf dem Gebiet des Knochen und Gelenkersatzes eine dauerhafte, moglichst lebenslange Implantation angestrebt. Seit Einftihrung des rostfreien Stahles in die orthopadische Chirurgie sind die durch die temporare Implantation verursachten biologischen Probleme geringer geworden. Die M erfolge auf dem Gebiet des dauerhaften Knochen und Gelenkersatzes haben jedoch die Implantatforschung intensiviert und ftihrten so zur Verbesserung der bisher verwandten Implantate aus Metallen und Kunststoffen sowie zur Entwicklung neuer Im plan tationsmaterialien. Die erste zusammenfassende Darstellung der Grundlagen der Alloplastik mit Metallen und Kunststoffen wurde 1967 von Contzen, Straumann und Paschke publiziert. Sie weisen auf das Hauptproblem der als Knochen und Gelenkersatz verwandten Implantate hin, niimlich auf die unzureichende Fixation des Implantates an das angrenzende biologische Gewebe. Grundsatzlich sind heute 3 Arten der Verbindung ImplantatImplantatlager denkbar (Hench u. Mitarb. 1971). Dieses sind: 1. die mechanische Bindung, 2. die Bindung in Abhangigkeit von physikalischen Kraften und 3. die chemische Bindung. Die bis heute iibliche mechanische Verbindung ihrerseits kann durch 3 Methoden erreicht werden: 1. die indirekte Fixation des Implantates durch Schrauben und Nagel, 2. die Verankerung des Implantates durch die Verwendung von Polymethylmethacrylat (PMMA)und 3. die direkte Impaktion des Implantates in den Knochen.
