Behandlung und Nachuntersuchungsergebnisse von 583 frischen Stauchungsbriichen am distalen Schienbeinende (davon 77 offene Frakturen) In den Jahren von 19261953 kamen im Unfallkrankenhaus Wien XX (Leiter: Prof. Dr. Lorenz Bohler) 330 intraarticuHire Stauchungsbriiche des distalen Schienbeinendes zur Behandlung und Beobachtung (davon waren 191 frisch und 139 veraltet). Mit Trojan habe ich diese Falle bearbeitet und wir konnten 214 Falle klinisch und rontgenologisch nach untersuchen. Wir haben damals versucht aus der mannigfachen Zahl der Brucharten einige charakteri stische Bruchtypen herauszuarbeiten. Nicht einbezogen wurden die Bruche beider Knochel mit Ausbruch eines groBen hinteren Schienbeinkeiles, die zu den Knochelbriichen gezahlt wurden. Aus den Schwierigkeiten, die bei der Behandlung auftraten, haben wir ModifIkationen in der konservativen Behandlung eingefuhrt und die Operationsindikation etwas erweitert. Diese Behandlungsart wurde dann im Unfallkrankenhaus Wien XII seit 1956 angewen det und lUckenlos einheitlich durchgefuhrt. Bis Ende 1974 kamen in diesem Krankenhaus 583 frische dista1e Stauchungsbriiche zur Behandlung. Davon waren 506 geschlossene und 77 offene Frakturen. Th. Ruedi, P. Matter und Allgower haben uber 84 Frakturen dieser Art, U. Heim und M. Nasen uber die Resultate von 128 Patienten berichtet. Von diesen Autoren wurde ausschlieBlich die stabile Osteosynthese der A. O. mit ihren Behandlungs und Nachbehandlungsmethoden angewendet. Wir haben 1976 un sere 583 Falle bearbeitet und nachuntersucht, urn herauszufinden, ob unsere uberwiegend konservative Behandlung noch Berechtigung hat. Besonders waren wir an Langzeitergebnissen dieser schweren Frakturen interessiert und wollten auch versu chen, das Arthroseproblem von den verschiedenen Seiten zu betrachten. A.
