Neben der Angina pectoris, dem Myokardinfarkt und dem plotzlichen Herztod, mit den en sich die koronare Herzkrankheit beim Patienten symptomatisch bemerkbar macht, ist in der letzten Zeit die stumme Myokardischamie vermehrt in das BewuBtsein interessierter Arzte geruckt. Zahlreiche Untersuchungen haben zum einen die Haufigkeit dieses Phanomens und zum anderen seine pro gnostische Bedeutung belegt, und es wendet sich das Interesse auch therapeuti schen Fragestellungen zu. Letztere mussen jedoch bezuglich einer Besserung der Prognose durch medikamentose Therapie noch als weitgehend offen angesehen werden. Neben anderen Forschergruppen, haben auch wir uns mit Problemen der stummen Myokardischamie beschiiftigt, "sind untergetaucht, um den Eisberg unter Wasser zu betrachten". Aus den Ergebnissen eines internationalen Sympo siums im Marz 1987 konnten wir ein Buch uber "Silent ischemia" in englischer Sprache herausgeben. Um deutschsprachigen Lesern einen kompakten und einheitlich gegliederten Uberblick uber Probleme und Erkenntnisse zur stummen Myokardischamie zu geben, wurde die vorliegende Monographie verfaBt. Sie dient dem Ziel, Interes sierte in wissenschaftlich fundierter, aber verstandlicher Weise in den derzeitigen Stand der Forschung einzufUhren. Mein Dank gilt an dieser Stelle meinem verehrten Lehrer und fruheren Chef, Herrn Prof. Dr. G. Riecker, der mir in dem wissenschaftlich liberalen und anspruchsvollen Klima seiner Klinik die Arbeiten ermoglicht hat, die in diese Monographie eingegangen sind.
