Frakturen des Os sacrum sind auch heute noch weitgehend vernachlassigte Verletzun gen, obwohl das Sakrum im Skelett eine besondere Rolle einnimmt, da es einerseits als Teil des Beckenrings eine wesentliche Bedeutung fUr die Beckenstabilitat hat, anderer seits als distaler AbschluB der Wirbelsaule eine direkte Beziehung zu den zentralen Ner venbahnen besitzt. Die Diagnostik ist bei den zumeist schwerverletzten Patienten sehr schwierig. Eine neurologische Untersuchung ist primar nur eingeschrankt moglich, und die Darstellbarkeit des Os sacrum auf Rontgenaufnahmen ist limitiert. Das fUhrt dazu, daB auch in neuerer Zeit noch bis zu 50% dieser Frakturen, selbst bei Vorliegen von Nervenschaden, ubersehen werden. Daraus resultierende, residuale Fehlstellungen im hinteren Beckenring sowie eine hohe Rate von Nervenschaden des Plexus lumbosacralis fuhren haufig zu klinisch und radiologisch unbefriedigenden Spat erge bnissen. Erst Ende der 80er Jahre konnten veriaBliche Zahlen zur Haufigkeit der Sakrumfrak tur gewonnen werden. Der routinemaBige Einsatz der Computertomographie verbes serte die Moglichkeiten der Diagnostik am hinteren Beckenring, auch eine Analyse der exakten Frakturmorphologie wurde nun moglich.
