Der grte Wirtschaftszweig mit rund 1,1 Millionen Beschftigten sind dabei die Akutkrankenhuser. Sie beanspruchen demnach auch die meisten Ressourcen im Gesundheitswesen. Seit 1972 werden Akutkrankenhuser in der Bundesrepublik Deutschland ber ein duales System finanziert. Abgelst wurde dabei das rein monistische Regime, dessen Wurzeln auf die Zeit um den 2. Weltkrieg zurckgehen. Hierbei wurden die Krankenhuser aus nur einer Hand, nmlich der staatlichen, finanziert. Im dualen System hingegen werden die Mittel der Betriebskosten auf der einen Seite von den Krankenkassen aus Leistungsentgelten, die Investitionskosten jedoch von Lnderhand aus Steuermitteln beglichen. Seit der Umstellung 1972 ist die Investitionsquote, also der Anteil an ffentlichen Frdermitteln an den Krankenhausausgaben von gesetzlicher und privater Krankenversicherung, von ca. 25 Prozent pro Jahr auf etwa 5 Prozent pro Jahr gesunken (s. Abb. 1). Vor dem Hintergrund der demografischen Entwicklung und des rasant wachsenden technischen Fortschrittes entwickelte sich ein erhebliches Missverhltnis zwischen dem tatschlichen Investitionsbedarf der Krankenhuser und den ihnen dafr zur Verfgung stehenden finanziellen Mitteln. Bis zum Jahr 2011 hat sich so ein Investitionsstau von 50 Milliarden Euro angehuft. Augenscheinlich wird das System der dualen Finanzierung den aktuellen Anforderungen und Bedrfnissen der Krankenhuser in Deutschland nicht mehr gerecht und wird in der Zukunft so nicht mehr zu halten sein. ber Alternativen wird seit Jahren diskutiert.
