Gesundheits und Pflegeberatung bei Hausbesuchen und Untersttzung mittels Case Management durch Pflegepersonen, sind Interventionen die im Rahmen des Projekts ?Seniorenberatung Tennengau? dafr genutzt wurden. Wissenschaftliche Evidenz ber prventive Hausbesuche in europischen Staaten, sowie die theoretischen Grundlagen der Methode des Case Management sind wesentliche Faktoren fr den Erfolg dieses in sterreich einmaligen Pilotprojektes. Die ber 80jhrigen zu Hause lebenden Einwohner in sechs Gemeinden wurden als relevante Zielgruppe fr die Hausbesuche identifiziert. Die Datenerhebung im Rahmen der Hausbesuche ist die Grundlage fr die vorliegende reprsentative Studie. Die biopsychosozialen Faktoren in Bezug auf die Aktivitten des tglichen Lebens und Instrumental Activities of Daily Living,die Wohnsituation und der Gesundheitszustand im Allgemeinen, Sturzhufigkeit, sowie der Beratungsbedarf werden dargestellt.
