Aprs un large rappel sur la pneumocystose, notre objectif tait dtudier la pneumocystose du nourrisson, par analyse rtrospective des hospitalisations en ranimation lhpital Trousseau (Paris) de 2001 2006. 60 enfants prsentaient une dtresse respiratoire aigu sans antcdent particulier, pour qui 11 lavages bronchoalvolaires (LBA) ont diagnostiqu 7 pneumocystoses associant fivre (4/7), polypne (7/7), toux (4/7), cyanose (4/7), bilan inflammatoire initial normal, et syndrome alvolointerstitiel diffus, et 4 coinfections. Tous ont t traits par oxygnothrapie, antibiothrapie, et corticothrapie, et 5 ont ncessit une ventilation mcanique. 3 enfants sont dcds, reprsentant 3/5 dcs parmi les dtresses respiratoires sans antcdent. Le bilan immunitaire montrait une lymphopnie CD4+ pour tous, 4 syndromes dimmunodficience combine svre (SCID), 2 sropositivit VIH, et tait normal pour 1 enfant. La pneumocystose doit tre recherche devant un nourrissons avec dtresse respiratoire aigu sans antcdent et syndrome alvolointerstitiel diffus. Le LBA est ncessaire pour confirmer le diagnostique et rechercher une coinfection.
