Der Entwicklungsstand der Organisation sowie des Informations und Planungs wesens in den deutschen Krankenanstalten bleibt derzeit weit hinter den Erfor dernissen eines modernen Managements zuruck. Beispielsweise verfugt der groBte Teil der Krankenhauser weder uber eine kaufmannische Buchfiihrung noch uber eine Kostenrechnung im betriebswirtschaftlichen Sinne; vielmehr begnugt man sich selbst in Krankenhausern mit jahrlichen Kostenbudgets von 10 bis 60 Mill. DM mit einem kameralistischen Rechnungswesen und erstellt nur die durch das Preisrecht vorgeschriebenen, wenig aussagekraftigen Selbstkostenblatter zur Berechnung der Pflegesatze. Das Planungswesen der Krankenhauser erstreckt sich in der Regel nur auf eine ein jahrige Ausgaben und Einnahmenrechnung. Wirtschaftlichkeitsrechnungen fur den Krankenhausbau, die Planung von Versorgungssystemen, das Bestellwesen von Ver sorgungsgutern aller Art wie z. B. Medikamente, medizinische Gerate usw. sind wei testgehend unbekannt. Der geringe Entwicklungsstand des Informations und Pla nungswesens im Krankenhaus gestattet es nicht, gesicherte Aussagen uber die Kostenwirkungen von Entscheidungen zu Machen. Die Unzulanglichkeiten im 1nformations und Planungswesen der Krankenhauser sind in erster Linie eine Folge schwerer strukturorganisatorischer Mangel im Kran kenhaus; Symptome fur die Mangel des Entscheidungsprozesses der Krankenhauser sind z. B. die weitgehende wirtschaftliche Abhangigkeit der Krankenhausfuhrung yom Trager, die geringen Entscheidungskompetenzen der Krankenhausleitung, die fehlende Zielformulierung sowie die unzulangliche Ausbildung der Fuhrungskrafte fur die Leitung von Krankenhausern.
