Se han estudiado un subgrupo de pacientes con Leucemia de Linfocitos Grandes Granulares con fenotipo CD4++/CD8+ los cuales presentan un receptor del linfocito T casi idntico en la regin CDR3 del gen Vbeta13.1. Adems, todos ellos comparten un alelo HLA de clase II DRB1 0701. Estos datos sugieren inequvocamente que estas leucemias son inducidas por un estmulo antignico. Al tratarse de neoplasias T de tipo CD4+, la naturaleza del antgeno causal deba de ser exgena al organismo humano, del tipo virus o bacteria. Dado que los pacientes estudiados presentan procedencias geogrficas diversas y comparten tanto el alelo HLA as como el tipo de familia del receptor del linfocito T, este agente desencadenante deba de ser ubicuo y extremadamente frecuente. Este estmulo patognico provoca la disregulacin del sistema inmune y la acumulacin de clulas reactivas (linfocitos T), con defectos en la homeostasis apopttica o muerte celular programada. La activacin inmune continuada de estos linfocitos y la aparicin de defectos en la maquinaria enzimtica que mantiene la integridad del genoma, incrementan las posibilidades de mutaciones genticas y por tanto de TRANSFORMACIN LEUCMICA
