La luxation congnitale de la hanche (LCH) est gnralement traite de faon prcoce. Cependant, le diagnostic aprs l'acquisition de la marche de cette affection n'est pas rare. Nous avons tudi 72 hanches prsentant une LCH idiopathique traites selon la mthode orthopdique de PetitMorel l'hpital Saint Vincent de Paul de 1975 2000.Une ostotomie pelvienne, pour corriger la dysplasie du cotyle, est la dernire tape du traitement. A plus de 10 ans de recul moyen, taient apprcies la congruence coxofmorale et lexistence d'une ostochondrite postrductionnelle.Les hanches sont classes Severin I ou II dans 95,8 % des cas. La complication principale est l'chec de la rduction avec reluxation, ncessitant une rduction chirurgicale (2 cas, 2,8%). Il existe un seul cas d'ostochondrite post rductionnelle, pour la seconde de ces deux hanches. Le traitement avant lge de 3 ans est un facteur de bon pronostic (p < 0.05). La mthode dcrite dans cet article semble tre celle de choix pour le traitement de la luxation congnitale chez l'enfant entre 1 et 5 ans.
