A diferencia de las LDL nativas, no modificadas, que poseen una una sola copia de Apolipoprotena B, las LPRs son el producto intermedio del catabolismo de la lipoprotenas de muy baja densidad (VLDL) de origen heptico conteniendo Apo B 100 y de los quilomicrones remanentes(QR), con apo B 48, respectivamente. Ambas aumentan en condiciones postprandiales, y tambin contienen Apo C. Las LDL deben oxidarse en el subendotelio vascular, donde son captadas por monocitos/macrfagos, que se transforman en clulas espumosas cuando LDL oxidadas entran va el receptor scavenger, que permite su acumulacin. En cambio, LPRs tienen sus sitios de oxidacin en los elementos formes sanguneos y en las clulas endoteliales. Ambas lipoprotenas son aterognicas pero LPRs activan el receptor LPOX1 de clulas endoteliales, abriendo brechas en el endotelio injuriado, permitiendo el pasaje de citoquinas y otros elementos aterognicos. Tambin son captadas por los macrfagos, sin previa oxidacin y promueven procesos inflamatorios con aumento de la Protena C reactiva (PCR de alta sensibilidad).
