La pandmie de linfection par le Virus de lImmunodficience Humaine (VIH) est lune des principales causes de mortalit dans le monde. Les combinaisons de traitements antirtroviraux, HAART (Highly Active Antiretroviral Therapy) ont fortement fait diminuer la mortalit et la morbidit lies linfection. Cependant, ces combinaisons antirtrovirales ne peuvent pas radiquer le VIH : il existe une reprise quasisystmatique de la rplication virale aprs arrt thrapeutique, et ce malgr des priodes prolonges de charge virale plasmatique indtectable. Cet ouvrage, objet dune thse dImmunologie en 2008 lUniversit Paris 7, fait le point sur les mcanismes de persistance du VIH chez les patients sous traitement efficace au long cours. Ces mcanismes sont tudis dans une catgorie particulire de cellules : les lymphocytes T rgulateurs. Ces cellules peuvent tre infectes par le VIH et sont capables dchapper leur destruction par les lymphocytes T cytotoxiques, en les inhibant. Ils constituent ainsi un rservoir privilgi du VIH. Ce travail explore des pistes thrapeutiques de rduction de ce rservoir particulier et propose une stratgie globale dradication du VIH.
