Die Pulmonalarterielle Hypertonie (PAH) ist eine schwere Erkrankung mit hoher Mortalitt, bei der es durch ein Zusammenspiel von Proliferation, Inflammation und endothelialer Dysfunktion zur Muskularisierung der Lungenarterien und einer Vernderung des Geftonus kommt. Die verminderte Bioverfgbarkeit von Stickstoffmonoxid (NO) wird dabei als ein wichtiges Krankheitsmerkmal angesehen. Der Auslser sowie die genauen Mechanismen der Erkrankung sind jedoch unbekannt. Myeloperoxidase (MPO) ist ein Hmprotein, das von Leukozyten freigesetzt wird und die Fhigkeit besitzt, im subendothelialen Raum zu akkumulieren. Dort oxidiert MPO endothelial generiertes NO. Whrend Studien die Bedeutung von Leukozytenaktivierung und MPOAktivitt fr Erkrankungen wie Vorhofflimmern und Linksherzinsuffizienz gezeigt haben, fehlen solche Daten fr die PAH und die chronische Rechtsherzinsuffizienz. In dieser klinischen Studie werden im ersten Teil die MPOPlasmaspiegel einer PAHPatientengruppe mit denen eines Kontrollkollektivs verglichen. Im zweiten Teil wird die Aussagekraft des MPOSpiegels auf die Krankheitsschwere und die Prognose der PAHPatienten in einer Verlaufsbeobachtung untersucht.
