Das multiple Myelom zieht mit seinem vielfaltigen und bunten Zellbild nicht nur begeisterte Zytomorphologen in seinen Bann, sondem fasziniert gleichwohl Internisten, Hiimatologen, Onkolo gen, Immunologen, Radiologen und moglicherweise auch Neuro cbirurgen und Orthop~den. Dieses breite Interesse ruft es sicher nicht zu Unrecht hervor, da wohl keine andere Erkrankung die im munbiologische Forschung mehr als das multiple Myelom befruch tet hat. Durch das Studium der Natur dieser Plasmazelldyskrasie wurden maBgebende Erkenntnisse iiber die Physiologie und Patho physiologie des immunkompetenten Systems gewonnen. So konn ten Plasmazellen vor allem aufgrund von Beobachtungen beim multiplen Myelom als antikorperproduzierende Zellen identiftziert und 2 bis dahin unbekannte Immunglobulinklassen (IgE und IgD) durch deren Vorkommen als Paraproteine bei Patienten mit multi plem Myelom entdeckt werden. AuBerdem konnte die Aufidwng der Aminos~uresequenz von Immunglobulinen bisher ausschlieB lich an Paraproteinen durchgeflihrt werden. Gegenw~g stehen Fragen beziiglich der klonalen Entwicklung normaler und neopla stischer Plasmazellen, der Regulation, Proliferation, Differenzie Plasmaze11k1onen, sowie die Inter rung und Wachstumskinetik von aktion mit anderen immunkompetenten Zellen, aber auch mit nicht verwandten Zellpopulationen, wie zum Beispiel Osteoklasten im Vordergrund des wissenschaftlichen Interesses. Daher ist in Zu kunft auf diesem Gebiet mit erweiterten Erkenntnissen zu rechnen, die moglicherweise bisher anerkannte Konzepte in Frage stellen und einzelne Abschnitte dieser kurzen Monograpbie iiberholen konnten noch bevor deren Drucklegung abgeschlossen ist. All die Bestrebungen, den Eigenschaften maligner Plasmazelldyskrasien auf den Grund zu kommen, werden von intensiven Bemiihungen zur Verbesserung der Therapie iibertroffen.
