Tachykarde ventrikulre Arrhythmien stellen die Hauptursache des pltzlichen Herztodes dar. Identifikation von Risikopatienten und adquate effektive antiarrhythmische Therapie sind daher von grBter Bedeutung. Systematische Studien zeigen, daS mit den derzeit verfgbaren Antiarrhythmika nur ca. 5060% der Patienten mit lebensbedrohlichen ventrikulren Tachykardien bei strenger Thera piekontrolle effektiv eingestellt werden knnen. Dabei sind in der praktischen Therapie nur wenige Antiarrhythmika verfgbar, die mit einer ausreichend hohen Responderrate einsetzbar sind. Andererseits stehen nichtpharmakologische BehandlungsmaSnahmen nur in begrenztem Umfang zur Verfgung, der klinische Stellenwert dieser Verfahren ist teilweise (z. B. Katheter ablation) noch nicht definiert. Es besteht somit ein Bedarf mr neue Antiarrhythmika, die sich in der Therapie lebensbedro hender Rhythmusstrungen als effektiv erweisen. Diprafenon, chemisch ein Propafenon Analogon, ist ein neues Antiarrhythmikum der Klasse le, we1ches ber ausgeprgte betasympa tholytische Eigenschaften verfgt. Der vorliegende Band enthlt eine erste Zusammenstellung der bisherigen experimentellen, pharmakokinetischen, pharmakodynarnischen und klinischen Befunde, die anlBlich eines Symposiums 1987 in Berlin zusammengetragen wurden. Obwohl mit den vorliegenden Daten eine endgltige Beurteilung des klinischen Stellenwertes von Diprafenon noch nicht mglich ist, erscheint die weitere klinische Prfung der Substanz sehr erfolgversprechend, insbesondere unter den Indikationen 'ventrikulre Tachykardie' sowie 'supraventrikulre Tachyarrhythmie' bei Prexzitationssyndrom.
