Bei metabolischaktiver Konkrementbildung der Nieren (Hypercalciurie und Harnsaure lithiasis) finden sich im Vergleich zu klinisch Gesunden eine Reihe von diskreten Unterschie den beziiglich der renalen Saureelimination. Unter allopurinolbedingter Harnsauresuppression sieht man einen Abfall der Titrationsaziditat, und bei Hypercalciurie findet sich deutlich ver mindertes Urinzitrat. Bei primarem Hyperparathyreoidismus sind diese Symptome starker ausgepragt, wiihrend Patienten mit Harnsaurelithiasis offenbar anderen Gesetzmiilligkeiten unterliegen, die vorwiegend iiber den Kohlenhydratund Fettstoffwechsel der meist adiposen Patienten gesteuert werden. Koch gilt die Durchblutungsstorung der Niere als wesentliches Merkrnal einer Steinentstehungskrise. Bekanntlich kann man aus den Ergebnissen der kon ventionellen Clearanceuntersuchungen gewisse Rtickschltisse auf die Nierendurch blutung ziehen. Ich mochte deshalb zuniichst Ergebnisse seitengetrennter Clearance untersuchungen bei Patienten mit einseitiger Urolithiasis mitteilen. Methodik und Durchflihrung seitengetrennter Clearanceuntersuchungen kann in diesem Kreis als bekannt vorausgesetzt werden. Zur Methodik mochte ich nur so viel sagen, daB wir die PAHund InulinClearance im sog. stady state bestimmen.
