Die psychosomatische Medizin bzw. Psychosomatik berucksich tigt neb en den somatischen auch die psychischen und sozialen Einflusse in der Atiopathogenese, in der Diagnostik, Therapie und im Verlauf der Krankheit. Beim einzelnen Patienten auch die psychosozialen Faktoren zu beachten, erfordert neben der personlichen Bereitschaft Kenntnisse und Erfahrungen in der psychosomatischen Medizin, Neurosenlehre und Psychotherapie und wie generell in der Medizin notwendig eine stete Fort und Weiterbildung. Diesem Ziel dienen die folgenden Beitrage, die ursprunglich als Vortrage an der Akademie fur Arztliche Fort bildung in der Arztekammer Berlin (Vorsitzende: Prof. Dr. Meta Alexander) als fachspezifische Fortbildung in der inneren Medi zin unter dem Leitthema "Psychosomatik" wahrend des Sommer semesters 1985 gehalten wurden. Die in dies em Band unter verschiedenen Aspekten beschrie benen Symptome und Syndrome lassen sich klinisch nach Herz KreislaufSystem, MagenDarmTrakt, Harntrakt und Muskel GelenkSystem zusammenfassen; danach folgen spezielle Syndrome, die erfahrungsgemaB groBe Schwierigkeiten in Dia gnostik und Behandlung bereiten. Am Anfang steht die Herzneurose, ein Krankheitsbild, das durch die hintergrundigeTodesangst immer wieder zu Noteinwei sungen und unnutzen EKGAbleitungen fuhrt. P. Bernhard bringt neben psychodynamischen und therapeutischenAspekten den wesentlichen Kenntnisstand zur Atiopathogenese, zu den Einflussen in der fruhen Kindheit und in der Erkrankungssitua tion, wobei er auch eigene Untersuchungsergebnisse darstellt und eindrucklich die Schwierigkeiten und Chancen der Arzt PatientBeziehung schildert.
