En los casos de avulsin dentaria en los cuales se producen daos extensos del ligamento periodontal por desecacin o almacenamiento inadecuado, el ligamento se necrosa y la cicatrizacin ocurre sin su intervencin. Gradualmente la raz es reabsorbida y reemplazada por hueso alveolar, la dentina radicular se fusiona al hueso y el proceso avanza en forma de una reabsorcin reemplazante ocasionando tambin una anquilosis. Se reporta el caso de un muchacho de 19 aos de edad que sufri, 8 aos atrs, la avulsin y la reimplantacin del incisivo central superior izquierdo. Fue evaluado clnica y radiogrficamente y se observ que el 21 presentaba los signos de una anquilsis y una reabsorcin radicular atpica. El diagnstico definitivo fue una reabsorcin radicular externa reemplazante. En este caso en particular la reabsorcin reemplazante ha superado el lmite de la cresta alveolar hasta penetrar internamente dentro de la porcin coronaria quedando el remanente coronario anquilosado. No se encontr en la literatura un reporte donde este tipo de lesin superase el lmite radicular.
