I bought this form design template to streamline my work and it was definitely helpful. However, I found myself a bit confused at times about how to customize certain fields. It took a few tries but once I figured it out, it worked great!

Decent, but not like new
I purchased this in used condition and while it works fine, I found a few pages were crumpled. It still does the job, just wish it was in better shape when it arrived. I guess you get what you pay for with used items.
